CPT® Codes For RPM Explained to Help for Your Practice – Part Four

Billing CODE 99454: Remote Patient Monitoring review of data

Your practice has chosen to develop your RPM and now your chronic care patients are supplied with connected devices such as smart scales, blood pressure monitors, glucose monitors and oximeters. Hopefully, each of those patients have received Remote Patient Monitoring training, from your staff or from an RPM business.  Each patient now knows how to use these medical devices.

It’s now time to use Billing 99454 for reimbursement for collecting the data.

Specifically, the Billing 99454 covers “Remote monitoring of physiologic parameter(s) (eg, weight, blood pressure, pulse oximetry, respiratory flow rate), initial; device(s) supply with daily recording(s) or programmed alert(s) transmission, each 30 days.”

As long as your patient steps on the connected smart scale once a day you will be reimbursed $64 every month for that patient.

It is important to note, that if you choose to use multiple devices, your reimbursement is the same. CPT® only permits one set of billing codes per patient per month regardless of how many devices they utilize.

As a physician, you know how difficult it is for patients to maintain a daily regimen. Just like with the patient onboarding process, Medek RPM works with your patients to ensure compliance.

As we have established a relationship with your patient onboarding process, we continually monitor your patients progress through our exclusive software programs.

Our team of medical care professionals works on your behalf with your patients. If there are any issues, we are quick to communicate with the patient, and when deemed needed, with your staff to solve those issues.

All of this is covered by Medicare, Medicaid and most private insurances.

Medek RPM can make sure any practice meets all the requirements of all CPT® codes. We have a team of professionals who stay on top of all the changes and work with a practice’s financial team to create the best financial return for their investment.

APCM Codes: The Quiet Revolution in Primary Care

APCM codes pay primary care providers for proactive, continuous care across all Medicare patients—not just in-office visits. Unlike CCM, they’re easier to bill and more inclusive. AI tools help practices meet APCM requirements by automating documentation, spotting care gaps, and supporting remote monitoring. The time to adopt is now.
